We are currently accepting new primary care patients, ages 3 and older. All patients of Self Integrative Care can expect the following:

  • A caring and helpful staff, comfortable waiting area, medical lending library, tropical freshwater aquarium, live plants, and complimentary beverages.
  • Discounted high quality supplements are available for purchase in our office and online, at our Wellevate Dispensary.

All insurance-based patients of Self Integrative Care can also expect the following, depending on an individual’s insurance plan benefits:

  • Appointments to Order and Review Lab Results
  • Appointments to Order and Review Medication
  • History & Physical (Routine Physical)
  • Annual Gynecological Exam
  • Well Child Check
  • Sports Physical
  • Urgent Care

  • Ear Irrigation
  • Nebulizer Treatment
  • In-House Lab Testing: Urinalysis, Pregnancy Testing, Quick Strep, and Occult Blood Stool Tests
  • Referral to Medical Specialists
  • Coordination of Alternative and Complementary Care
  • Telephone Access to On-Call Provider for Urgent and After-Hours Care Coordination Medical Specialists
  • Comprehensive and Focused Skin Exams
  • Treatment of Skin Lesions with Liquid Nitrogen
  • Biopsy of Suspicious Skin Lesions

Being aware that it may take some time to get an appointment with a local dermatologist and that patients are often alarmed at the long wait, I have done additional training to be able to assess suspicious skin lesions and offer skin biopsy, to be able to serve patients in a timely manner. - Carolyn Self
