Why membership?
Functional Medicine Memberships allow an approach not limited solely to diagnosis-driven care.
What will my membership cover?

Will my insurance cover my membership fee?
No. This is an “out-of-pocket” service not covered by insurance.
Do you prescribe medication?
Our memberships do not include prescribing or management of chronic medications.
How do I become a member?
Once Carolyn has reviewed your membership questionnaire, our office will communicate with you directly by phone or email to ensure that a membership will best serve your needs.
How do I pay for membership?
Upon membership enrollment, you will be provided details for our office to make arrangements to bill your credit card.
What is the cost of membership?
Cost of Membership is based on Type of Membership. Currently, I offer 2 Options:
- 3 Hour Membership=$950
- 6 Hour Membership=$1800
Typically, your first appointment is 60 min and includes a review of Medical Records: (Chart Notes, Labs, Diagnostic tests and Procedures) done prior to the initial consult.
