Client Area of Interest Membership

Client Area of Interest Memberships provide education and counseling consultations, designed to help you become a healthier, happier person. These memberships are best suited to individuals looking for an in-depth exploration of a particular area of their health, allowing for time to be spent in an educational approach to a health concern.

All clients of Self Integrative Care can expect:

A caring and helpful staff, comfortable waiting area, medical lending library, tropical freshwater aquarium, live plants, and complimentary beverages.
Discounted high quality supplements available for purchase in our office and online at our Wellevate Dispensary.

Your membership begins with:

An initial 60 to 90-minute face-to-face consultation
Review of prior medical records, focusing on improving your health
Ordering and review of labs, diagnostic tests, and procedures, specifically related to your area of interest
Coordination with complementary and alternative medical providers
Research on topics you request
Telephone or e-mail correspondence with parties you approve

After your first visit, membership services may be provided in person, by telephone, electronic messaging, or via “Tele-Medicine”, for 30 to 60 minute appointments.

As a membership client, you’re able to schedule appointments online, at your convenience, using our Electronic Health Record Portal, which gives you 24-hour access to your medical records.

Membership consultations require the member to maintain a primary care provider to prescribe medications, do routine exams, as well as urgent care. If diagnosis is needed to help cover the cost of specialized testing for insurance billing, membership clients may schedule a regular medical office visit with Carolyn to determine appropriate medical coding. These office visits are billed to contracted insurance companies, or are “out-of-pocket”.
